Responsibilities: 

·        Bridges direct patient care and healthcare administration. AS Program Manager / Care coordinator, you assess patient needs, develop care plans, and manage facility operations—such as scheduling, budgets, and staff supervision—while adhering to provincial nursing standards

Requirements: 

·        LPN with CLHA, College of Licensed Practical Nurses and Health Care Aides

·        (3) years Licensed Practical Nursing experience, preferably within senior’s care home environment including dementia/geriatrics
